Abstract The article discusses research which was conducted to investigate a pharmacist managed anticoagulation model clinic in a patient centered medical home at the Rosa Parks Wellness Institute for Senior Health (RP-WISH) to determine whether the model provided safe and effective anticoagulation which was cost effective. Researchers evaluated the cases of 246 patients. They found that the model provided anticoagulant management that was safe, effective and resulted in financial savings.
