Abstract The article discusses an initiative by the American Academy of Ambulatory Care Nursing (AAACN) to improve care transition between patient-centred medical home providers and outpatient and community settings. The care coordination competency action plan consists of 3 phases; phase I involved the creation of an action plan, phase II was about developing competencies for care coordination while phase III was to review the competencies developed in phase II.
