Catalogue of Federal PCMH Activities

The Catalogue of Federal PCMH Activities summarizes the PCMH-related work of Departments and Agencies participating in the Federal PCMH Collaborative (see description below). The Collaborative created this Catalogue so that people can find out easily about the PCMH work being done by participating agencies. The Catalogue also provides information about ways the agencies are working together to promote a strong primary care delivery system, which includes supporting promising models of primary care transformation like the PCMH.

About the Federal Patient-Centered Medical Home (PCMH) Collaborative and the Catalogue of Federal PCMH Activities

Federal Collaborative

The Agency for Healthcare Research and Quality convenes a collaborative group of Executive Branch employees in Federal agencies and departments that are currently doing work related to the PCMH or have an interest in the PCMH.

The Collaborative provides opportunities to increase cooperation and collaboration on PCMH-related activities and minimize duplication of effort among Federal agencies. It also provides an opportunity for Federal personnel in the Washington, DC, metropolitan area to develop a common base of knowledge about the PCMH model through in-person presentations by colleagues and by non-Federal experts.

Developing the Catalogue Summaries

To gather information for the Catalogue, AHRQ and Mathematica Policy Research staff conducted telephone discussions in August and September 2010 with individuals from agencies participating in the Federal PCMH Collaborative. From these conversations, they created summaries of activities for each agency and a table that compares activities across all agencies. Each agency updated the project information in March/April 2011, and reviewed and approved its summary before the information was posted to the AHRQ PCMH Web site.

Maintaining and Updating Catalogue Content

The agency summaries are maintained by members of the Federal PCMH Collaborative Steering Committee and updated several times each year.
